Remnant roots of crossbite anterior teeth are rare, and are often treated with fixed dentures after extraction of residual roots in clinical practice. In the past 6 years, the author has used the combined orthodontic and prosthetic treatment method to preserve such residual roots for post-core crown restoration, and has achieved good results.
1 Materials and methods 9 patients, 9 residual roots of crossbite anterior teeth, including 3 maxillary central incisors and 6 maxillary lateral incisors. The youngest is 13 years old and the oldest is 25 years old. The selection criteria were: maxillary crossbite anterior residual root, no percussion pain after complete root canal treatment, no obvious loosening, root surface at least flush with the gingiva, and enough space to accommodate the residual root in the dental arch. Treatment method: First, a curved “?”-shaped temporary short post is bonded through the residual root canal for orthodontic traction. Install the square wire arch bracket to fix the segment appliance. The arch wire should be made of titanium-nickel wire or titanium-nickel wire plus stainless steel double arch wire. Get enough space to accommodate the stump. The remnant of the crossbite was pulled into the normal dental arch through titanium-nickel wire and “?” shaped nails. Finally, the temporary short post was pulled out, and the residual root was repaired with conventional cast post and core and porcelain crown.
2 Results The residual roots of 9 crossbite anterior teeth in 9 patients were treated by combined orthodontic and restoration, and the aesthetic and functional requirements of the anterior teeth were restored. 6 cases of 6 teeth, after 2 months to 4 years of follow-up (the other 3 cases were lost to follow-up): the anterior teeth were arranged normally, the porcelain crowns and residual roots were not loosened, there was no porcelain chipping, there was no obvious inflammation of the gums, and there was no obvious inflammation of the gums. The dental adjacency was normal and the patient was satisfied.
3 Discussion The preservation treatment of residual roots and residual crowns has preserved a large number of teeth that were considered to be extracted in the past for patients, and has achieved great success [1]. However, due to its abnormal position, the residual root of the crossbite is often extracted in clinical practice, and the missing tooth space is repaired with a fixed bridge. We use the orthodontic method to pull the dislocated residual root into the normal dental arch, and then repair the post and core and porcelain crown, retain the root, avoid tooth extraction, and avoid the shortcomings of grinding and cutting adjacent teeth when the fixed bridge is repaired. . Combined orthodontic and restorative treatment proposes a new method for the preservation and treatment of residual roots of crossbite anterior teeth. For adjacent teeth with mild misalignment, segmental arch appliances can also be used to correct and align them at the same time. .
